I am struggling with the design of the tables and forms for a database that will be replace a safety survey-type card. Users are expected to complete this card twice a day. There are several (30+) standard 'checks' (things to gauge for safety) and each one should be responded with either safe or unsafe or not applicable. It has to be one of these choices!

I am trying without much success to provide a single form that shows all of these 'checks' and the 3 options beside them, for the user to simply click on their choice.

One of my assumptions is that these 'checks' will change over time. The client will add or delete them. I want to be able to accomodate for this.

Any help would be greatly appreciated, such as a sample or resource on a web-site to help explain a general approach. If anyone would be willing to correspond off-line with me I would provide more specifics such a sample.

What version of Access are you using? Please remember to select the version number when posting questions in case it becomes relevant to the discussion.

That would not be easy to do, and I don't think you can avoid redesigning the form if the business process change in the future.

Here's a demo in the Code Archive by NoahP that sort of shows you how to use multiple checkboxes on a form.

Hope it gives you some ideas for your project.

This link - Library of Database Models , specifically Number 31, may be along the lines of what you're looking for. When on the web page, use Ctrl+F and type in "Survey" to find other similar models.

When I completed my posting I made sure that I answered the version at the top of that screen! Where did it go?

I'm using Access 2007, or Office 2007.

For future reference, if you "preview" your post before submitting it, you will have to re-select the version number or it will not show up.
