Click on the File tab, then select Options (second from the bottom on the left).
Select Current Database (second from the top on the left). Application Icon is the second item down. |

I use to have icons into a subfolder within the app folder and I set it via vba so the icon can be shown even if you take your folder app to another location. I insert a combobox with a list of the icons and give the user the chance to choose which icon to set. I may leave an example if you will.
