I used access to create a form, which worked perfect. There was 1 submittion for each, so it couldn't hold multiple records, however I tried adding a second record anyway and now my report will not show anything. Hopefully I can attach the file for someone to work with. I would like to be able to add multiple records and each one just show up on a new report page, but I will be happy if it just works so I can input the data and see it on the report.
