I believe you were trying to attach a database to your post.
To do that follow these directions.

Attaching Images/Files

When adding a new (or replying to a) post, images and/or files may be attached by using the "Attachments" section tools. They are found immediately beneath the post dialog box and you can provide as many images/files as you wish; providing they do NOT exceed 2Meg in size total. Using this tool allows a poster to upload images/files directly from their harddrive.

NOTE: When attaching non-image files, you MUST "zip" them first.
NOTE 2: When attaching a zipped Access database, PLEASE do a Repair & Compact BEFORE zipping them.

I see 6 other people have downloaded your database, so lets see what suggestions they give you, and I will look at it also.

Are you trying to make the report look like the one you posted?

The database you posted only has the tables in it have you tried to create any queries or reports at all (just need to know)?
