I have a fax form that is linked to our phone contacts database, and what I am trying to do is when an individual enters his name in the name field of the fax form, his/her phone and fax # will populate the phone and fax field.

Can someone tell me how?

Easiest way is to use a combobox for the user to select their name rather than enter it.

Use the wizard to create a new combobox on your form and select the third option (which says something like 'Find a value' etc) and follow the prompts...........
