Good Morning UA,

I'm fairly new to Access & UA but I have been browsing for awhile. I have searched the site up and down to find a way to make the survey database I'm working on fit my needs. I'm trying to create just a basic questionnaire that questions if a task has been completed or not by a specific person. I would also like to create reports to track the data entered.

I believe I have a pretty normalized structure. My problem is with my forms. I am code "illiterate" dunce.gif and I just don't know how to setup a form properly to pull the info needed from my tables. Can anyone help me? I have attached the file below. I understand the way I have the form setup now isn't correct but I wanted to give an example of what I'm trying to accomplish.

Good morning and welcome to UtterAccess.

Unfortunately, your zip file contains only the ldb, which is the locking file for the database, not the database itself.

Please try again, making sure you upload only the mdb.

Also, have you looked Duane Hookom's "At Your Survey" for ideas?

Thanks for the help George! I have attached a new copy hopefully it will work this time. I have tried "At Your Survey" and although I think it's awesome it wasn't quite what I needed for this specific project.

Hello, I've been looking at this database as I find time (sorry, it's been a very busy couple of days).

I don't know quite how to proceed. There is a large amount of work to do with this before it can be really useful. I feel bad at having to say this , but my assessment is that most of what you have now will have to be revised. I will upload a demo later, with some changes that reflect a more normalized design.

I guess the good thing is that you are at the early stages, where you still can make changes to the tables.

If someone else with more time wants to take a look and offer further suggestions, that will be good too.

Okay, I'm going to upload the partial suggestions I have, but I am leaving on vacatiion and can't really finish it out. Hopefully we'll get some help. Here's the approach I think you need to take, with a more normalized table design and a set of forms to support them. I did figure out a way to preserve the check boxes, but it will NOT prepopulate the form with them. That will take a bit more code trickery.
