I would like to have a custom form for reports which has page navigation
buttons, like First, Previous, Next, and Last

Okay, it's a report. I was assuming you were working with a continuous form. (Which is what I recommend if the display is not for printing.)
What does your navigation popup form do that the navigation buttons at the bottom of the report don't?
